| Obverse: |
The coat of arm of Johannesburg with antelope supporters, lion crest and no motto. In an arc below: “JOHANNESBURG”. The S.A.M mark appears at the foot on the gold and silver medals.

In the centre, an arched stone bridge through which passes a road with a large numeral “5” along its surface, leading to mountains beyond. On the keystone of the bridge, a springbok head and the date: “1965” split left and right by the head. On a scroll above following the arch of the bridge on the left: “REPUBLIEK VAN / SUID-AFRIKA” and on a similar scroll on the right: “REPUBLIC OF / SOUTH AFRICA”.
